Bias Incident Reporting Form:
Please use the form below to report an incident of possible bias, hate or discrimination. A team coordinated by the Director of Equity, Diversity and Inclusion will review the form. This team will review the incident, advocate and empower those affected, inform and educate the campus community, and promote civility and respect. Incidents reported anonymously may limit the University's ability to respond to the situation.
Note: If your concern is related to an EMERGENCY where there may be an imminent risk to the safety of others, this form is not the appropriate way to report your concern. Instead, please contact one of the following:
